Drive my elbow into my side to prevent over the top?

There's an angle between your left forearm and the clubshaft (I'm assuming you play right-handed). Hold that angle for the split second you start the club down. Then just keep that angle, not holding on for dear life, but in a relaxed way and it will release itself by the time the clubhead gets to the ball. Do a lot of swinging, with no ball in front of you, slowly and gracefully, to start making this your habit.
 
I like the "no ball" idea. Putting a ball there introduces the "hit impulse"! This is another way of phrasing "don't hit AT the ball" , "swing THROUGH the ball"! Just swing your swing. Whatever happens, happens.
