Help with shanking pitch shots

Over the last few rounds I have been shanking the heck out of pitch shots and I cannot figure out a cure. It is costing me anywhere between 5-6 strokes a round. Any helpful hints to assist me in killing these?

Well thanks to youtube I think I figured it out. In the yard at least.. We'll see how it goes on the course..
