Blew up a brand new Adams Speedline driver tonight. How??

I had a Nike driver do this once about 7 years ago. I'd had the driver for a week maybe, and on the range the head flew off in my back swing and almost took my coach's head off. It's no fun at all when this happens.

Happened to my mizuno mx700 driver this year. Sent back to mizuno and new shaft and grip fitted.
